Ankylosing Spondylitis (AS) is a chronic inflammatory arthritis primarily affecting the spine and sacroiliac joints, leading to pain, stiffness, and reduced mobility. Major symptoms include chronic back pain, morning stiffness, fatigue, and progressive spinal fusion. Extra-articular problems may involve the eyes (uveitis), heart, and lungs.

The Janus kinase (JAK) pathway plays a role in the pathogenesis of AS by mediating inflammatory cytokine signaling, especially involving IL-17 and IL-23. JAK inhibitors, a newer class of targeted therapy (JAKi), can help reduce inflammation and disease activity in AS, offering relief for patients who do not respond to conventional biologics like TNF inhibitors.

Clinical studies have shown that Upadacitinib can greatly reduce back pain and stiffness in Ankylosing Spondylitis. By week 14, nearly 44.5% of patients treated with Upadacitinib experienced noticeable symptom improvement, compared to only 18.2% of those receiving a placebo.
